Vall d’Hebron implements One Step Ictus: pioneering technology and approach to streamline and improve stroke care

One Step Ictus transforms care for patients with ischaemic or haemorrhagic stroke by concentrating the entire care approach in a single direct-access room, reducing response times from arrival at the Emergency Department to the start of treatment.

In stroke treatment, time is brain. And it is precisely for that – saving time – that the implementation of One Step Ictus was intended. This pioneering, world-first project introduces a new way of managing the stroke code and makes it possible to save time, which can be crucial in reducing the neurological after-effects suffered by stroke patients. One Step Ictus is, then, a new technology accompanied by an equally innovative approach.
 
This project was implemented in 2023. One Step Ictus required the refurbishment of the interventional neuroradiology unit, which was equipped with cutting-edge technology to enable the Neurology and Neuroradiology teams to assess, diagnose and treat patients more quickly.
 
Thanks to the One Step Ictus project, patients with stroke symptoms are taken directly to the One Step Ictus room instead of following the traditional Emergency Department circuit, using a corridor specially designated for transferring these patients. The pioneering technological solution in the One Step Ictus room combines biplane angiography to perform mechanical thrombectomy, the standard procedure for removing the clots that cause stroke, with CT equipment to carry out scans before, during and after the thrombectomy, and to assess the patient’s cerebral blood flow. This approach doubles the likelihood of a good functional prognosis for patients with more severe strokes: it is estimated that 48% can achieve an almost complete clinical recovery after undergoing direct thrombectomy, compared to 27% of patients who follow the traditional circuit.
