En alguns càncers, la propagació del tumor és impulsada per un rar nínxol de cèl·lules mare marcades amb SRY (regió determinant del sexe Y) -box 2 (SOX2). A causa dels seus papers clau en el manteniment del creixement del tumor, proporcionant quimio-resistència i impulsant la recaiguda del tumor, és fonamental dilucidar els mecanismes subjacents que propaguen aquestes cèl·lules SOX2+ per desenvolupar teràpies dirigides. Les anàlisis de dades de seqüenciació de cèl·lules unicel·lulars realitzades en el tumor cerebral maligne pediàtric més comú, el medul·loblastoma (MB), van revelar l'existència d'un clúster astrocític enriquit amb Sox2 que expressava biomarcadors de senyalització Sonic Hedgehog (SHH). Curiosament, el tractament amb un inhibidor de SHH que actua sobre Smoothened (Smo), vismodegib, va provocar augments de cèl·lules semblants als astròcits que expressen Sox2, així com els efectors SHH aigües avall Gli1/Gli2 que suggereixen la seva resistència a la inhibició de Smo. Utilitzant cultius MB enriquits amb SOX2, vam observar que les cèl·lules SOX2+ no només expressaven proteïnes GLI, sinó que requerien la seva senyalització per propagar-se. A diferència de la massa tumoral altament proliferativa, en què la senyalització SHH es desencadena principalment per mutacions en components SHH, a les cèl·lules MB SOX2 +, la via SHH es va activar aigües avall de Smo de manera dependent de MYC. És important destacar que la inhibició de la senyalització SHH mitjançant inhibidors de GLI funcionalment diferents va esgotar cèl·lules SOX2 + resistents a vismodegib de teixits MB derivats de ratolí i humans, reduint la seva capacitat per empeltar encara més in vivo i augmentar la supervivència sense símptomes en models de medulloblastoma resistents i sensibles al vismodegib. Els nostres resultats posen l'accent en la importància d'utilitzar teràpies dirigides a GLI per esgotar les cèl·lules SOX2+ i, per tant, proporcionar una remissió tumoral estable.
