El tractament amb el fàrmac levodopa permet recuperar moltes de les funcions deteriorades o perdudes a causa de la malaltia. És el més eficaç, però alhora és un tractament que necessita millores per què en avançar la malaltia el seu efecte es fa transitori i fluctua; quan actua la medicació el pacient es troba bé, en "On". I quan se li acaba l'efecte es troba en "Off" i els símptomes reapareixen.
Per millorar els efectes del levodopa s'ha investigat en administrar-la per vies diferents (inhalada, dèrmica, intrajejunal) i en diverses formes farmacològiques. Ha estat especialment exitosa l'administració a través de gastrostomia en una infusió de gel de levodopa.
Hi ha altres tractaments farmacològics i neuroquirúrgics, com estimulació elèctrica de parts del cervell, que ofereixen bons resultats. S'està treballant a incidir en el cervell malalt, a través d'estereotàxia amb ultrasons, estalviant la trepanació i cirurgia.
S'han obert noves vies d’investigació en la malaltia de Parkinson per a esbrinar-ne la causa, evitar-ne la progressió i millorar al màxim els seus símptomes.
