Health and Social Work

The Health and Social Work Unit attends and responds to social or psychosocial situations that affect the optimal functioning of the patient in their own healthcare or that of their family members, whether in situations brought about by a certain health problem or social situations deriving from said condition.

The unit accompanies the patient or family member throughout the illness, assessing, advising and informing them on all social aspects that may affect their health, with the aim of promoting social well-being and ensuring a better quality of life.

Aims

The main objectives of the Health and Social Work Unit are as follows: To identify and assess social or family needs and problems; to intervene in social or family problems in order to recover or preserve balanced social behaviour by activating the necessary formal or informal social support; to work on situations of vulnerability and fragility; to provide social assessment and social healthcare diagnosis; and to plan future proposals for social work action.

Functions

Its basic functions include healthcare, coordination, prevention and health promotion, as well as research, teaching, strategic planning and effective and efficient distribution of social resources.

    • Monday to Friday, from 8 a.m. to 3 p.m.
  • Functional team of child maltreatment experts
    • Monday to Friday, from 8 a.m. to 3 p.m.
  • Emergencies
    • Monday to Friday, from 8 a.m. to 10 p.m.
    • Saturdays and Sundays, from 8 a.m. to 8 p.m.

If you find yourself in any of the following situations, we can offer you support:

If a family member has been admitted to hospital or you have had to move from your area and currently find yourself in a situation of social vulnerability, we can offer advice about accommodation providers close to the hospital.

Helping the patient and their family members to better understand the illness through the lens of their social and family reality, and to reorganise family roles based on the given social and healthcare needs.

We advise and inform you about the procedures to be followed in order to access public services and grants, based on your health situation and needs or those of your family member.

We provide mediation services in situations that may generate conflict, thus guaranteeing the wellbeing of the patient during hospital admission.

We plan the hospital discharge by taking into account the personal resources of the patient and his/her family members and, where appropriate, by activating the social welfare and health network of the corresponding area.

The intercultural mediation service is offered to facilitate communication between our professionals and patients and to improve their understanding of the diagnosis and treatment.

Intercultural mediation

The Intercultural Mediation Service is a resource offered to users from different socio-cultural backgrounds. The socio-health mediator is the member of staff who acts as a bridge to facilitate the relationship between healthcare professionals and immigrant users, helping to create a setting of trust that ensures equal and normalised access to healthcare services.

The intercultural mediation service is available in three languages:

  • Arabic (Khoukha, Wednesday, from 9 a.m. to 3 p.m.)
  • Chinese (Tian, Tuesday, from 9 a.m. to 1 p.m.) 
  • Urdu (Madhla, Thursday, from 8.30 a.m. to 12.30 p.m.)
